Eligibility & Appointment Information
If you have Medicaid, Medicare, Medicaid Managed Care, charity care or you do not have insurance, please call the main number to be directed to the correct appointment desk. An appointment must be made in order to be seen in any of our services. If you are a member of a Medicaid Managed Care HMO, the primary care physician must be a doctor within the Family Health Center for you to be eligible for primary care services. There is a charge for all services, however, if you do not have insurance, you can set up an appointment or interview with a financial counselor to complete a financial evaluation at 732-902-7080. Having the following documents will greatly assist in the financial evaluation process:- Proof of income (three months of pay stubs)
- Proof of address (driver’s license or envelope showing your current address)
- Social Security card
- Any type of insurance card
Checking and savings account statements
A complete evaluation of the financial evaluation and determination process will be provided during your interview. While in the process of applying for Charity Care, patients can schedule an appointment. Medicare & Charity Care Patients: The New Jersey Department of Human Services has instituted an insurance program called “Family Care.” You may become part of an HMO. The Fulfill mobile pantry is at the Jane H. Booker Family Health Center every third Wednesday of the month from 2 p.m. - 3:30 p.m. Individuals can visit the pantry to receive non-perishable food, as well as fresh produce and baked goods.Partnership with Sustain & Serve NJ
We are proud to offer fresh meals to the community Monday through Friday between 10 a.m. - 11 a.m. and 2:30 p.m. - 3:30 p.m. All individuals are welcome and can simply visit our location during the established times to receive a free meal. Upon your arrival, please notify a staff member that you would like to request a meal.
